Info & Policies


DFP staff is available Monday through Friday, 8:30 AM to 5 PM to make you an appointment at a convenient time and at the location nearest to your home or work. Please assist us in appropriate scheduling by notifying us 24 hours in advance if you cannot keep your scheduled appointment.

Medication Refills

To have a medication refilled, please ask your pharmacist to call our Pharmacy Refill Number at 816-889-1530, Choice 1, leaving your name, medication, date of last refill, and quantity. This will enable us to promptly respond to your request. Refills can be requested Monday through Friday 8:30 A.M. to 4:00 PM. It is our policy to not refill medication on the weekends or after 4:00 PM.

Medical Records

Your medical records are confidential. We do not release routine records without your written consent. Click here to view our HIPAA policy.


Our physicians are on rotating call 24 hours per day, seven days a week including holidays.

Telephone Calls

During office hours, we almost always have a nurse available to answer any questions you may have. If we are to return your call, please remember to leave your area code and telephone number to ensure timely response. Telephone calls may be delayed when the physician is in surgery; however, you will be notified if we anticipate a delay and another nurse or physician may be available to address your question.

Disability Forms

Due to the number of requests we have for completing disability forms, we ask you complete your portion and sign the claim form. Enclose a self-addressed stamped envelope for return to you. Limited staff is available to complete these forms and there is a $20.00 prepayment per form before completion. We ask for your cooperation.


We will gladly assist you in filing your insurance claims. Please advise the front desk upon your arrival of any insurance changes. This will assist in getting timely response from your insurance carrier. We participate in a number of insurance plans. Due to the complexity and volume of plans we participate in at DFP, it is impossible for us to know the specifics of your plan. You are responsible for knowing these specific requirements of your particular insurance. We cannot negotiate in settlement with your carrier over disputed claims, medical-legal situations or divorce, as we are not a party to those contracts.

Payment Policy

Our billing office number is 816-889-1540 and is open for your convenience 8:30 AM to 5:00 PM Monday through Friday. Co-pays must be paid at the time of your arrival. If we do not participate with your insurance company, payment is expected at the time of your appointment. We accept Master Card and Visa for your convenience. Our insurance and collection department will assist you in setting up a payment plan or discuss problems you may be having with your insurance company. We are willing to assist you whenever possible.
